SHIPPING INSTRUCTIONS

Samples can be shipped using the United States Postal Service (USPS), FedEx, or United Parcel Service (UPS).

What To Ship:

Samples

a.  Bundle tubes together in groups of 2-10 tubes with a rubber band

b.  Place inside a leak proof zip lock bag

c.  Wrap the bags with newspaper or paper towels (enough to absorb all fluid if tubes break).

d.  Pack the bags inside a cardboard box insulating with bubble wrap or absorbent newspaper.

e.  It is not necessary to ship samples on ice.  If you plan to hold samples before shipping, store samples in refrigerator.

Sample Submission Form

a.  Enclose a completed sample submission form with the samples. The submission forms can be downloaded from the website or by contacting us directly.

Payment:

a. Check, money order or credit card.

c.  $3.00 per unit (Price includes blood sample tubes and needles)

How To Ship:

USPS

a.  Write Biohazard on the zip lock bag or on a note size piece of paper with a sharpie.

b.  DO NOT PUT BIOHAZARD ON THE OUTSIDE OF THE BOX!

c.  Write "Exempt Animal Specimen" on the outside of the box.

d.  If you have questions, please consult with your local post office.

FedEX

a.  Write "Exempt Animal Specimen" on the outside of the box.

b.  If you have questions, please consult with your local FedEx store.

UPS

a.  Write "Exempt Animal Specimen" on the outside of the box

b.  If you have questions, please consult with you local UPS store
